Health Plan Terms & Conditions
- This is not an insurance plan or product. Services for this membership plan are only offered at Dr. Mindy’s Family Dentistry and not at any other dental office, including specialists.
- This membership plan is for patients with no dental insurance.
- Coverage is in effect for 12 months after your membership registration date. Payment in full is required. There is no reimbursement for early termination of membership plan, if patient acquires dental insurance, or for plan preventative services not received. Unused services cannot be carried over to the next plan year.
- Fees for dental services are due when services are rendered. Late payment will result in loss of offered discount. Treatment plan will be provided in writing for all recommended procedures displaying the regular fee and the discounted fee.
- 15% courtesy can only be used towards dental services. Products including, but not limited to, Prevident toothpaste, whitening products and electric toothbrushes are not eligible for any discounts.
- Appointment times are limited, and it is the responsibility of the patient to schedule appropriate visits.
